Insurance Waiver Form – Note: Business Office must receive proof of coverage by September 1 (February 1 for new spring semester enrollees); otherwise students will be automatically enrolled in the Student Insurance Plan and the premium ($288 per year/$193 for spring semester enrollees) will be added to the student’s account.
